EI-JSK, Gulfstream G650, Westair Aviation

At 16.30 hrs on 7 July 2019, the aircraft departed Shannon Airport (EINN) with two pilots, one cabin attendant and one passenger on board. Approximately 15 minutes after take-off, while climbing through Flight Level (FL) 290, an amber 'L [Left] ENGINE MAINTENANCE' message appeared on the Engine Indicating and Crew Alerting System (EICAS). This was quickly followed by an Electronic Engine Controller (EEC) commanded shutdown of the left-hand engine with an associated red 'L ENGINE FAIL' message on the EICAS. The right-hand engine reverted to an alternate control mode. The Pilots declared a PAN, which was later upgraded to a MAYDAY, and diverted back to EINN where they landed safely. The crew and passenger disembarked normally. There were no injuries.

The Bolkow Bo-209 Monsun aircraft, which was fitted with a retractable nose landing gear, took-off from Runway 09 at Navan Airfield (EIHH) with two occupants on board. The nose landing gear was retracted after take-off, as normal. Following one circuit of the airfield, a landing on the same runway was carried out. During the landing roll, the propeller and the lower engine cowling made contact with the surface of the grass runway, as the nose landing gear had not been extended prior to landing. The aircraft's propeller and engine cowlings sustained damage. The two occupants were uninjured and exited the aircraft unaided. There was no fire.

The aircraft was on a flight from Weston Airport (EIWT), Ireland, to Abbeyshrule Aerodrome (EIAB), Ireland, with two persons on board. Whilst landing on runway (RWY) 10 at EIAB the aircraft left the paved surface of the runway and came to rest on the grass verge, to the right of the runway. The Pilot and passenger exited the aircraft uninjured. The Pilot reported that the aircraft was not damaged. The aircraft was subsequently flown back to EIWT.

Interim Statement: The aircraft took off from Runway (RWY) 29 at Kilrush Airfield (EIKH), Co. Kildare, at approximately 18.02 hrs UTC, and was to land back at EIKH upon completion of the flight. The aircraft occupants’ families became concerned when the aircraft had not returned, and contact with An Garda Síochána was made that evening. The aircraft wreckage was located by the Dublin-based Search and Rescue helicopter (R116) at 03.29 hrs. The aircraft had impacted in a grass field, located approximately 5.2 nautical miles (NM) to the south of EIKH. The aircraft was destroyed. The two occupants were fatally injured. There was no fire.

The Piper PA-28R-180 aircraft, with a Pilot and one passenger on board, landed on Runway (RWY) 29 at Sligo Airport (EISG). During the rollout, approximately 150 metres after touchdown, the aircraft experienced what the Pilot described as ‘a strong bump’ which coincided with the right wing dropping slightly. The Pilot contacted air traffic control and was instructed to park the aircraft on the airport apron. It was subsequently discovered that the aircraft suffered a partial collapse of the right-hand main undercarriage. The occupants of the aircraft were uninjured. There was no fire.

The microlight was on a local flight from a private airstrip at Rathcash East (Gowran), Co. Kilkenny, and was airborne for three to four minutes before the accident occurred at approximately 20.30 hrs. The Pilot was observed operating at low level in the area before proceeding back towards the airstrip. The microlight collided with one of two wooden poles supporting electrical power lines close to the private strip, and fell to the ground. The Pilot was fatally injured. There was no fire.

The aircraft, with a solo Student Pilot on board, was carrying out circuit training at Waterford Airport (EIWF). As the aircraft commenced a turn onto the base-leg of its first circuit, the engine began to vibrate violently, lost power, and did not respond to throttle inputs. On final approach to Runway (RWY) 21, the Student Pilot declared an emergency (Mayday), and informed EIWF Air Traffic Control (ATC) that he was carrying out a forced landing. The aircraft touched down in a field, bounced, and continued through a boundary hedge. The aircraft came to rest, inverted, in the adjacent field. The Student Pilot exited the aircraft uninjured. The aircraft sustained substantial damage. There was no fire.

The aircraft was on a scheduled passenger flight from Birmingham (EGBB) to Malta (LMML). During descent, the Cabin Services Supervisor (CSS) was advised by the Commander that turbulence may be encountered while deviating around weather. The Commander also switched on the fasten seat belt sign and passengers were seated. Sudden turbulence was encountered while descending through Flight Level (FL)180. Three Cabin Crew Members (CCM) were at the rear of the aircraft at the time, securing the rear galley and toilets. All three fell to the floor and CCM No. 2 sustained serious leg injuries. No other injuries to the passengers or crew on board were reported to the Investigation.

G-KNYS, Cessna 208B, Parachuting Caravan Leasing Pty Ltd

The Cessna 208B aircraft took off from Runway 27 at Clonbullogue Airfield (EICL), Co. Offaly at approximately 13.14 hrs. On board were the Pilot and a Passenger (a child), who were seated in the cockpit, and 16 skydivers, who occupied the main cabin. The skydivers jumped from the aircraft, as planned, when the aircraft was overhead EICL at an altitude of approximately 13,000 feet. When the aircraft was returning to the airfield, the Pilot advised by radio that he was on ‘left base’ (the flight leg which precedes the approach leg and which is normally approximately perpendicular to the extended centreline of the runway). No further radio transmissions were received. A short while later, it was established that the aircraft had impacted nose-down into a forested peat bog at Ballaghassan, Co. Offaly, approximately 2.5 nautical miles (4.6 kilometres) to the north-west of EICL. The aircraft was destroyed. There was no fire. The Pilot and Passenger were fatally injured. The Investigation determined that the probable cause of the accident was a loss of control in a steeply banked left-hand turn, leading to a rapid loss of altitude. Four Safety Recommendations are made as a result of this Investigation.

Having just completed a local flight consisting of circuits at Cork Airport with a Flight Instructor, the Student Pilot was conducting his second solo flight, which also consisted of circuits at the airport. While attempting to land during the second circuit, the aircraft bounced on the runway a number of times and then landed on the runway, nose wheel first. This resulted in the partial collapse of the nose landing gear, with consequent damage to the aircraft engine and propeller. The Student Pilot exited the aircraft uninjured. There was no fire.
